Here on Pokemon Celes we don't ask for much in terms of the character creation process. You are free to change the layout of the application however you wish, so long as the following information is found on it in some distinguishable way.
You can include a picture if you want but you still need the typed description. <:

[b]Gender:[/b] Woop!
[b]Age:[/b] Self Explanatory
[b]Height:[/b] In feet and inches, please.
[b]Weight:[/b] In pounds, please.
[b]Region:[/b] The region they are from.
[b]Description:[/b] At least a good paragraph. It's really not that hard I promise.
[b]Personality:[/b] At least two good paragraphs!
[b]History:[/b] At least two good paragraphs! Should include how they became a breeder/trainer/etc. How they got to Celes if not native, how they got their starter, etc.
